same form with a new signature to request samples every calendar
month.
1-877-882-5302 - Allergen - You must call every month and request a
new form be faxed to you. You should expect the fax within 1 business
day.
1-800-323-0000 - Bausch & Lomb - They will fax you a form that you can
recycle with a new signature once a month.
